TH1 Protocol:
The Infection connection to chronic illness:

It has become apparent that an underlying cause of a number of chronic ailments is the presence of hidden bacterial infection. A significant number of bacteria are capable of changing their structure from the typical forms described in medical texts, i.e. rods (bacilli), spheres (cocci) or spirals (spirocites), to atypical forms such as L forms (cell wall deficient) and cysts. Bacteria capable of this pleomorphism include those found in ticks, fleas and mosquitoes. They are responsible for Lyme and other associated tick borne diseases as well as bacteria that cause pneumonia and sexually transmitted diseases.

As these bacteria transform into divergent forms they inhabit certain white cells; cells which are unable to kill them. They then take over some of the cell's metabolism and provoke an unregulated inflammatory response. This increased inflammation cloaks the bacteria and prevents the production of antibodies - antibodies which are vital in the elimination of offending bacteria and in the development of long term immunity.

At the intracellular level, these bacteria grow very slowly and their toxic effects often progress over many months and/or even years. The increased and uncontrolled inflammation persists affecting many organs in the body including the muscle-skeletal system, thyroid and other hormone related glands, skin, brain and the immune system.

The symptoms associated with both the excess inflammation and the bacterial toxins closely resemble the symptoms of:

• chronic fatigue
• fibromyalgia
• environmental hypersensitivity-allergy syndrome

If this infection continues, profound neurological problems can arise resulting in:

• poor memory
• poor decision making
• anxiety
• depression
• sleep disturbance
• tremor
• numbness
• tingling and weakness

Diseases believed to be manifestations of this condition include:

• Bell's palsy
• trigeminal neuritis
• interstital cystitis
• irritable bowel syndrome
• psoriasis
• sarcoidosis
• heart block
• restless leg syndrome
• Lou Gehrig's disease
• rheumatoid arthritis
• tourette's syndrome
• autism
• multiple sclerosis

Diagnosis:

Because the bacteria evade the immune system, blood tests are commonly negative, although elevated cholesterol is often found. Therefore, diagnosis stems primarily from clinical history and findings. Bacteria associated with tick bites should be screened, even though the tests normally done by the CDC are quite insensitive. More sensitive tests are available through specialized labs in the USA. A blood test to check for abnormal levels of Vitamin D will indicate inflammation present. Basically, infected white blood cells up regulate the conversion of inactive Vitamin D. Thus, by measuring both active and stored Vitamin D levels, and watching for either an increase in the amount of, or an increase in the ratio between the two forms of Vitamin D - this, will confirm the presence of a TH1 mediated inflammation.

Vitamin D related complications:

Vitamin D is a pro-hormone, not a vitamin. Typically, it is produced in the skin by the conversion of cholesterol via UV radiation. Excess Vitamin D is toxic and is not found normally in most foods. It is estimated that only 30 minutes of sun exposure to the face and arms is sufficient to produce all the Vitamin D needed in a light skinned person. With TH1 induced inflammation, too much Vitamin D aggravates the inflammation and interferes with the effects of therapy.

Treatment - the 3 pillars of treatment are:

• Reduce TH1 induced inflammation
• Reduce bacterial load
• Support detoxification

1) Reduce the level of inflammation

This is necessary as the increase in TH1 mediated inflammation inhibits the ability of the immune system to identify the pathogen and generate the appropriate antibodies needed to effectively kill the bacteria and prevent recurrences. Also, reducing the inflammation lessens the damage to organs and tissues and the symptoms arising from that inflammation.

2) Reduce bacterial load

Certain antibiotics, used in lower than normal doses and frequency, effectively kill intracellular bacterial pathogens. When this occurs, the white cell containing the bacteria also dies. This results in an increase in toxins and causes a temporary increase in symptoms. This reaction is called a Herxheimer reaction. Herx reactions are the insurance needed to indicate that the bacteria are dying off, and their level of intensity guide individual therapeutic choices. The idea is to dose the amount and frequency of the medications to create a controlled Herx reaction.

Typical Herx symptoms:

• Digestive: increased frequency of heartburn, gas, loose stool, cramps, constipation
• Kidney-Bladder: increased frequency of urination, prostatic inflammation, transient decrease in libido, erectile dysfunction, menstrual irregularity, painful periods
• Fatigue: this is common however, most patients enjoy an improved and deeper sleep
• Skin: itchiness, pimples, rashes, increased intensity of pre-existing skin problems
• Headache, Muscle and joint pain, tired muscles
• Lightheadedness
• Palpitations, chest pain: these can be worrisome but are generally benign.

3) Support detoxification

• Hydration: Drink plenty of fluids, 2 to 4 liters per day
• Rest: Allow for plenty of time to rest and/or sleep
• Diet: Avoid Vitamin D containing foods; eat a low carbohydrate diet and especially low in sugars and refined carbohydrates; eat adequate protein; eat organic foods as much as possible and as unprocessed as possible; you may increase your salt intake if you are experiencing lightheadedness.
• Sunlight: Though variable, most patients will benefit from reducing their exposure to sun and bright light, especially early on in the process.
• Exercise: This is totally up to each individual.
